To apply for fiscal sponsorship, sometimes called fiscal agent, the organization(grantee) must submit a two page proposal, explaining your project in detail. Include the following:
- Narrative Project description prepared by Grantee
- Detailed project budget and recent financial statement
- Timetable of activities (as appropriate)
- Description of lead project staff for Grantee
- Evaluation procedure
- Grantee must obtain State of Colorado non-profit application or certificate, and
- Copy of completed IRS Form SS-4 OR IRS 501©(3) determination letter
- Grantee’s Board of Directors
Upon receipt of the above items, the Community Foundation will make a determination to proceed with the Agreement between the Foundation and the Project. Foundation staff may require additional information on a case-by-case basis.
